Please be sure that your primary physician has provided a referral before your initial appointment with us, if required by your insurance provider.
If you require a follow-up appointment, we encourage you to schedule that appointment before you leave your current visit. Every effort will made to schedule your appointments in a convenient manner. Later, if you find you need to reschedule, we would appreciate your calling the office as soon as you know that you cannot keep the appointment. A missed appointment is a loss for everyone.
